Can anyone give me any information on this piece? I am in Ontario Canada but not sure where it is from.

That seems to be a custom made, unless you see some indication of numbering, tag, logo or a short product details. You can recreate are replica of that f you want, you just need to contact any woodworking and furniture shop, or try it here http://caldwells.com/door-shop, they are manned with expert furniture maker. They also sells different kinds of doors.
